Description
Position assists with all aspects of evaluating patients for admission in Rehab. Responsibilities including planning, development and follow-up of an appropriate plan of care for patients in need of extended care placement or home care following discharge from the skilled care unit.

Qualifications




Education:

* Graduate of an RN school
* Certifications:
* Maintain Basic Life Support (BLS/CPR) Certification.
